Our mission is simple: help individuals and families find the healthcare services and resources they need before a health crisis occurs. We connect people to the care they already qualify for — at no cost, with no judgment, and no bureaucratic runaround.
We believe that everyone deserves access to clear information, affordable healthcare options, and support in making informed decisions about their health.

Many Texans face real barriers to care.
Across Texas, individuals and families face barriers such as lack of insurance, transportation challenges, language differences, limited healthcare providers, and difficulty navigating complex healthcare systems.
These challenges are often greater in rural and underserved communities — where distance, resources, and information gaps can turn a small health concern into a serious crisis.
Community Health Access Hub was created to bridge the gap between available healthcare resources and the people who need them.
We exist so that families have somewhere to turn — and someone to help them find the right care, at the right time, close to home.
