Texas Medicaid document checklist
Gather these before you open the application. You can upload photos taken with a phone — they don't have to be scans.
Photo ID for the adult applying
Texas driver license, state ID, passport, or school ID for a teen applicant.
Social Security numbers
For everyone applying. If a number was requested but not issued yet, bring the receipt.
Proof of Texas residency
A lease, utility bill, or any official mail with your name and Texas address.
Proof of income for the last 30 days
Pay stubs, or a signed letter from your employer with hours and pay rate.
Self-employment records
Last year's tax return, or a simple written profit-and-loss for the last month.
Proof of pregnancy with a due date
A clinic note or lab result. Needed for Pregnancy Medicaid and CHIP Perinatal.
Immigration or citizenship documents
Only for the people applying — not for other household members.
Bank statements and proof of resources
Required for seniors 65+ and applicants applying on disability (MEPD).
Proof of other insurance, if any
Employer plan, Medicare card, or a termination letter if coverage just ended.
Child care, child support, or medical bills you pay
Some costs are deducted and can bring you under the income limit.

Read the reason first — most denials come from missing paperwork, not income. You can appeal by the deadline in the letter (usually 90 days), or simply reapply with the missing proof. A navigator can review the letter with you for free.
Can Texas Medicaid cover bills I already received?+
Often yes. Texas Medicaid can be backdated up to three months before the month you applied if you would have qualified then. Tell HHSC about unpaid medical bills from that window while you apply.
