Request help
Tell us how we can help
Free and confidential. A community navigator usually reaches out within 1–2 business days.
This is not emergency medical care. If you or someone you know is experiencing a medical emergency, please call 911 right away.
Not sure who to call? Describe what you need
Tell us what's going on in your own words and we'll suggest local organizations from the directory below that may be able to help. Nothing you type is saved to your request — please leave out full names, dates of birth, and Social Security numbers.
Common questions about getting help
- How fast will someone contact me after I request help?
- Most people hear back within one business day by phone, text, or email — whichever you choose on the form. If your situation is urgent, call 911 for an emergency or dial 211 any time for immediate Texas referrals.
- Which Texas counties do you serve?
- We focus on Brazoria County and the surrounding area, including Fort Bend, Galveston, Harris, Matagorda, Wharton, San Jacinto, and Polk counties. If you live elsewhere in Texas we will still point you to the closest statewide options.
- Do I need insurance or immigration papers to ask for help?
- No. You can ask for help whether or not you have insurance. We never ask for a Social Security number on this form, and being uninsured or unsure about your immigration status does not stop you from getting clinic, food, or pregnancy support.
- Can you help me apply for pregnancy Medicaid or CHIP?
- Yes. Navigators walk you through pregnancy Medicaid, CHIP, CHIP Perinatal, SNAP, and Marketplace applications, help you gather documents, and follow up if your application stalls.
