Who qualifies for each program?

Community Resource Navigation

Who it's for: Any resident of Brazoria, Fort Bend, or San Jacinto County needing help navigating healthcare, food, or benefits.

  • No income test required
  • No insurance status requirement

Senior Support Navigation

Who it's for: Adults 60+ and their caregivers in our three-county area.

  • Age 60 or older
  • Any income level
  • Any insurance status
  • Caregivers of seniors are also eligible
  • In-home and phone navigation available

Maternal & Family Support

Who it's for: Pregnant women, new mothers, and families with children under 19.

  • Pregnant at any stage
  • Children under 19 in household
  • Families with income up to 200% of Federal Poverty Level typically qualify for CHIP
  • WIC open to pregnant/breastfeeding women and children 0–5
  • No documentation required to receive navigation help

Rural Health Access Initiative

Who it's for: Uninsured and underinsured residents in rural areas of our three-county service area.

  • No insurance or inadequate coverage
  • Rural or remote ZIP codes

What programs does Texas Health Help offer?+
Texas Health Help offers five free programs: Community Resource Navigation (connecting families to healthcare, food, and benefits), Senior Support Navigation (Meals on Wheels, Medicare counseling, caregiver support), Maternal & Family Support (WIC, CHIP, Medicaid for pregnant women), Rural Health Access Initiative (clinic navigation for uninsured rural Texans), and Community Outreach & Education (health fairs and resource events).
Do I need insurance to get help from Texas Health Help?+
No. All Texas Health Help navigation services are free and do not require insurance. We specifically help uninsured and underinsured families connect to free and sliding-scale clinics, benefits programs, and community resources.
Who qualifies for senior support navigation?+
Any senior 60 or older living in Brazoria, Fort Bend, or San Jacinto County can receive free navigation help. We help seniors find Meals on Wheels, Medicare counseling (SHIP), senior transportation, caregiver support, and medication assistance programs. No income test required for navigation services.
How does the Maternal & Family Support program work?+
Our navigators help pregnant women and families apply for WIC, CHIP, and Medicaid. We also connect families with postpartum support, parenting resources, and food assistance. Services are free, confidential, and available in English and Spanish.
