Change Happens! deploys certified community health workers into Houston neighborhoods rather than waiting for people to come to an office. CHWs do home and community visits, help manage chronic conditions like diabetes and hypertension, and hand-walk residents into clinics and behavioral health services.
Best for: People managing a chronic condition who want a person, not a phone tree.
